Perioperative anemia is an independent clinical risk factor associated with increased morbidity, mortality, and transfusion requirements. Its early detection and management are fundamental components of preoperative evaluation. In oncologic, neurosurgical, and cardiac patients, preoperative anemia worsens postoperative outcomes and prolongs hospitalization. The current approach is based on Patient Blood Management programs, which emphasize early diagnosis, etiologic correction, and rational transfusion practices. Strategies such as intravenous iron supplementation, erythropoiesis-stimulating agents, antifibrinolytics, and blood conservation techniques enhance perioperative safety and outcomes. We provide an overview of perioperative anemia management.
